Some 20 members of Deltalinqs were recently introduced to the incident analysis method Tripod Beta. The master class was organizedin part by DGMR to acquaint companies in mainport Rotterdam with this research method. Kuiper & Burger provided the master class because of our years of experience with incident investigations.
The mini-training given by colleague Marco van de Pavoordt focused on all kinds of incidents, both large and small, as well as near-misses. This makes the Tripod Beta method a widely applicable tool, also used by the Dutch Safety Board, for example. The diverse group of participants of lawyers, safety experts, SHEQ specialists and port pilots were explained interactively how an incident analysis works and why Tripod Beta is such a good and powerful method.
Participants considered different research methods, objectivity in research and the added value of Tripod Beta as a tool that reveals more than just the (near) failure. Paying close attention to underlying causes, Marco emphasized that analysis is not just about pointing out causes. It is primarily about learning from mistakes so that they can be prevented in the future. Good incident analysis is perhaps the best way for an organization to improve.
